For Incoming Patients
We recommend filling out the HHHRC Intake Form prior to your appointment.
However, if you are unable, please arrive 15 minutes before your appointment to complete this form.
Co-Pay Update
Since our opening, we have not collected payments for medical services from our clients in an effort to provide low-threshold, affordable health care. However, to remain in compliance with State and Federal laws, HHHRC is required to begin collecting co-pays effective July 1, 2024. Please see the letter below for more information.
