Hawaiʻi Health and Harm Reduction Center (HHHRC) is thrilled to share the incredible progress and supportive atmosphere cultivated at Hoʻokahi Leo Kauhale, a low-barrier tiny home shelter program focused on building intentional community. This unique initiative provides stable housing and comprehensive services designed to empower residents and foster meaningful connections.
At Hoʻokahi Leo, we are deeply committed to enhancing our residents’ lives through various services that address their immediate needs while paving the way for long-term success. Below are some of the exceptional programs and assistance offered by our dedicated staff:
Comprehensive Services Offered
Medical Services: HHHRC’s Medical Mobile Unit frequents the site to provide basic services and works with residents to establish care with a primary care and mental health provider.
Transportation Services: Residents can access transport to medical appointments, errands, and shopping, ensuring they stay connected to essential services.
Individualized Workforce Development: Our staff works together with residents to help them find and prepare for employment. This includes job selection, applications, resume building, interview coaching, and even providing appropriate interview clothing.
On-Site Donation Closet: A crucial resource, the donation closet is stocked with essential items that residents may need, allowing them to feel comfortable and supported.
Short-Term Bus Pass Program: Through the HOLO card program, we assist residents with temporary transportation access, empowering them to eventually navigate independently.
Cell Phone Support: We provide short-term cell phone and data card support to help residents manage their communication needs as they transition to independence.
Celebrating Special Moments: Each resident receives a personalized birthday cake on their special day, fostering a sense of community and belonging.
Community Meetings & Activities: Our weekly sit-down dinners and community gatherings provide opportunities for residents to connect, share meals, and engage with one another.
Zumba Classes: To promote physical health and wellness, we offer weekly Zumba classes, making fitness fun and accessible for everyone.

Personalized Care Coordination

Community Outreach & Housing Navigation: Our staff assists residents in navigating available resources and securing housing options.
Individual Case Management: Each resident may work with a Peer Navigator who provides case management services and conducts regular assessments, develops personalized service plans, and provides ongoing monitoring and follow-up.
Accessing Healthcare & Support Services: We facilitate access to medical care, mental health support, and substance use services, ensuring residents can address their health needs effectively.
Crisis Intervention: Our trained staff is equipped to provide crisis intervention and support, helping residents navigate challenging situations.
Empowering Residents Through Education & Employment
Education is a cornerstone of our program. We offer various workshops and classes covering essential life skills, healthy boundaries, mental health awareness, and more. Participants are encouraged to engage in their learning and share this knowledge with their families and support systems.
In an exciting new development at Hoʻokahi Leo Kauhale, we have launched a groundbreaking initiative that empowers residents by hiring them as contracted peer security aides. This program plays a vital role in enhancing safety within our community and provides residents with invaluable work experience, fostering their skills and confidence in a supportive environment. By integrating residents into the security team, we promote a sense of ownership and responsibility, enabling them to act as protectors and advocates for their peers. This peer-driven approach ensures that the security measures reflect the unique needs of our community while cultivating mutual respect among residents.
The peer security aide program also serves to reinforce the personal growth and transformation of those involved. Participants receive training that equips them with conflict resolution skills, emergency response techniques, and community engagement strategies, ultimately preparing them for future employment opportunities within the broader workforce. This role enriches their resumes and instills a sense of purpose and accomplishment. As they protect and serve their fellow residents, they forge stronger connections and create a culture of support and collaboration within their community. Together, this initiative exemplifies our commitment to empowering individuals, building relationships, and fostering a safe and thriving community for all.
Success Stories: A Testament to Change
Stetson’s Journey
After experiencing homelessness for over 15 years in the Sand Island area, Stetson moved into Hoʻokahi Leo Kauhale in February 2024. Since then, her life has dramatically improved. She reconnected with family on the Big Island and cherished visits over the holidays.
Stetson has made strides in managing her health and has successfully reduced her substance use. She is currently thriving as a contracted peer security aide with HHHRC, gaining valuable work experience and saving for her own home. Her active participation in Zumba classes and her use of our Medical Mobile Unit highlights her commitment to health and well-being. The weekly community dinners have allowed her to build friendships as she steps outside the comfort of her previously introverted nature.
Daryl’s Transformation
Daryl’s story is equally inspiring. After living unsheltered for over 20 years, his move to Hoʻokahi Leo Kauhale in February 2024 marked a pivotal moment in his life. Initially hesitant, Daryl embraced the stability offered and began taking control of his health. For the first time in years, he sought medical attention and successfully managed his diabetes, with regular doctor visits becoming part of his routine.
Daryl has joyfully reunited with his brother, who now visits him regularly, instilling a sense of belonging that he long sought. He has found a peaceful, supportive environment that meets his needs, allowing him to flourish and embrace this new chapter in his life. Daryl’s story speaks to the transformative power of compassionate care and the supportive environment fostered at Hoʻokahi Leo.
